Successes in Medicaid Delivery & Population Health Transformation:
A Case Study of Staten Island and How The Results Can Work Elsewhere

Delivery System Reform Incentive Payment (DSRIP) programs and other Medicaid Delivery Transformation Section 1115 Waiver programs are designed to expand Medicaid eligibility and clinical services for low-income residents in participating states.  This program will present a case study that illustrates how the Staten Island Performing Provider System (SI PPS), one of the 25 PPS in New York, is achieving its DSRIP project goals, and what similar healthcare organizations Massachusetts and other states, including ACOs, MCOs, and other DSRIP and Medicaid Delivery Reform programs, can learn from them. In particular, we will focus on SI PPS’s initiatives in the following areas:  Managing ER use; Reducing preventable readmissions; Aligning behavioral health and primary care; Utilizing social determinants of health data; Managing opioid addiction and substance abuse; Patient attribution; Working with Community Based Organizations. Faculty will specifically help determine how successful strategies, processes and lessons learned from New York DSRIP can be applied across Medicaid Reform Initiatives in Massachusetts.
